Delivered across Australia by General Practice Networks, Lifestyle Modification Programs (LMP) are a Commonwealth Government funded initiative, targeted at the the prevention of Type 2 Diabetes.

LMP facilitators must be accredited

The Murrumbidgee General Practice Network (MGPN) is delivering the Reset Your Life LMP.

The Reset Your Life LMP provides motivation and education in:

  • the risks of diabetes and their relationship to lifestyle risk factors;
  • the importance of regular diabetes screening;
  • nutrition advice and education;
  • physical activity advice;
  • behavioural strategies to support the adoption and maintenance of healthy lifestyle changes;
  • smoking cessation and alcohol reduction advice if required; and
  • information about community resources to provide support in maintaining lifestyle

The MGPN has 6 accredited program providers in the communities of Leeton, Narrandera, Griffith, Hay, Hillston and Finley. The LMPs are held at a variety of locations and range of days and times, specific to each town.

The program is delivered face-to-face in group sessions. There is a minimum of 8 hours contact time, with an initial intensive phase of at least 4 months. A follow-up session at the end of the program occurs at least 6 months after the commencement of the program.

Patients are eligible to take part if they:

  • Are aged 40-49 years, or Aboriginal or Torres Strait Islander aged 15-54;

             &

  • Have a risk of type 2 diabetes based on the AUSDRISK tool – score 12 or higher;

The MGPN is working with local practices to help practice nurses and practice staff identify eligible patients.

A GP referral is required for eligible patients to access the Reset Your Life LMP.

GPs send the LMP referral form centrally to the MGPN, who then notifies the appropriate accredited provider to enrol the patient as a participant. 


NB/ Patients who have diabetes or have previously taken part in a subsidised LMP are not eligible.
